A health plan sought to reimagine primary care but faced misalignment across payers, providers, and quality frameworks. The work focused on ecosystem mapping, incentive alignment, and cross-payer coordination — resulting in the California Advanced Primary Care Initiative, a shared payment and quality model adopted across multiple organizations. Moved from research to design and implementation of a groundbreaking pilot
Impact: Enabled a first-of-its-kind pilot involving 12 primary care practices and 3 payers covering thousands of lives – operating under a unified set of quality measures and incentives, fostering consistency and shared goal.
